- Title - title of your paper in bold, upper and lowercase letters, centered in the upper half of the page
- Author's Name - double space under the title, not bolded, same font size
- Institutional Affiliation - school name, double space underneath author's name, same font as above
- Course Name - double space underneath the school name, same font as above
- Instructor Name - double space underneath the course name, same font as above
- Assignment Due Date - double space underneath Instructor's name, same font as above
- Page Number - put number 1 on the top right hand corner of the header on this page
